Required Forms

The YOUTH CODE OF BEHAVIOR must be completed by every student 12 years and older. One form per child. Email the form to Ms. Riley Oliver at [email protected] or bring it in on the first day of camp.

A YOUTH PROGRAM EMERGENCY MEDICAL INFORMATION AND RELEASE 25-26 must be completed, one form per family.

The Youth Program Emergency Medical Information and Release 23-24 form must be filled out every year starting July 1. The single form is applicable for each ministry listed on the form. It does not need to be duplicated. One form per family.

Grades 5-8
For young pianists, early intermediate and up.
Registration fee $200/person*
Monday - Friday, June 23-27, 1:30pm-4:30pm.
Friends and family welcome to attend on Friday for performances.
